Job SummaryWe are seeking a highly motivated and empathetic Community Outreach Specialist to join our team. As a key member of our outreach team, you will play a critical role in connecting with patients, informing them about our services, and helping them take the first step towards improving their health. Key Responsibilities- Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies to educate them about our services and encourage them to enroll.
- Develop and maintain a personal caseload of patients, ensuring that each assigned patient is contacted using the most effective strategy.
- Enroll patients into our program, collecting key data about patients during the enrollment process.
- Interact with enrolled patients to ensure ongoing engagement and deliver additional community-based touchpoints as needed.
- Participate in weekly team meetings focused on ongoing education and improvement.
- Accurately and timely document patient interactions and outcomes in our company's software system.
- Excellent verbal communication skills and ability to establish trust with patients.
- Persuasiveness and ability to clearly communicate our service offering and value.
- Active listening skills and genuine compassion for others.
- Quick thinking and ability to respond to questions and objections.
- Organized and self-motivated, with ability to work independently and meet established goals.
- Ability to work collaboratively within a team environment and with people of diverse skills and experience.
- Knowledge of community resources in the local market.
- Able to maintain clear professional boundaries with patients and coworkers.
- Commitment to representing the company with professionalism and cultural competency.
- Comfortable using computer for documentation, communication, and organizing work.
- Must have reliable transportation to perform essential outreach functions.
- Must be fluent in English; Spanish speaking a plus.
- Able to work flexible hours, including occasional night/weekend work.
- Minimum of 3 years of work experience, preferably in sales, healthcare, or outreach.
