POSITION SUMMARY: Our Assertive Community Treatment (ACT) team is in search of a compassionate professional to provide around-the-clock community support for individuals experiencing severe and ongoing mental health conditions. As a Community Support Specialist, you will work closely with clients and their support systems to develop tailored recovery strategies that cater to their medical, psychological, social, educational, and vocational requirements. Your responsibilities will include aiding with daily tasks, crisis management, and linking clients to essential community services.

Key Responsibilities:
  • Support individuals in daily living activities and resource access
  • Advocate on behalf of clients with external organizations to obtain necessary support
  • Provide crisis intervention and emotional support
  • Deliver direct services to clients while ensuring all documentation is completed
  • Implement strategies aimed at preventing out-of-home placements and minimizing the need for intensive services
  • Efficiently manage a caseload and maintain precise documentation
  • Participate in team meetings and ensure adherence to service standards
  • Maintain confidentiality and comply with HIPAA regulations
  • Perform additional duties as required
Qualifications

ESSENTIAL QUALIFICATIONS:
  • Education - Bachelor's Degree in Human Services or a related discipline
  • Licensure/Certification - Not applicable
  • Experience - At least 2 years in direct care and electronic record-keeping
  • Other - Clear criminal background check, valid driver's license, drug screening, TB screening
  • Bilingual abilities are preferred
PREFERRED QUALIFICATIONS:
  • Experience - Minimum of 4 years in a human services-related field and electronic documentation
