Client Support Specialist

We are currently in search of a Case Manager to join our RISE Program. This role involves comprehensive case management for clients and meticulous record-keeping for those engaged with the RISE initiative. The Case Manager will report directly to the Program Coordinator.
Salary InformationHourly Rate: $27.47
Key Responsibilities:- Deliver direct client services, including outreach and assessments via videoconference in correctional facilities, intake, engagement, service plan formulation, referrals, advocacy, counseling, exit summaries, and follow-up.
- Manage a diverse caseload effectively.
- Track and document client progress towards established service plan objectives.
- Maintain accurate records of all client interactions.
- Stay informed about available program resources both on-site and in the community.
- Coordinate services with various social service, mental health, medical, employment, educational, and other providers.
- Facilitate case conferences with clients, service providers, and legal representatives as necessary.
- Accompany clients to court hearings when required.
- Draft brief update letters for the Court regarding client progress.
- Engage in bi-monthly meetings with the supervisor to report on case management activities.
- Ensure compliance with all relevant regulations and organizational policies.
- Provide crisis intervention and stabilization support as needed.
- In-person collaboration with colleagues is essential.
- Perform additional duties as assigned.
Minimum Requirements:
- Bachelor's degree in criminal justice, social work, psychology, or a related field.
- Alternatively, a high school diploma or equivalent with a minimum of three years of progressive case management experience in the criminal justice sector.
- At least three years of relevant case management experience.
- Strong interpersonal skills and attention to detail are crucial.
- Excellent verbal and written communication, time management, and organizational abilities are required.
- Familiarity with community resources is advantageous.
- Ability to address problems, make informed decisions, resolve conflicts, and actively listen.
- Capacity to remain calm and effective in crisis situations.
- Proven ability to collaborate successfully with diverse groups.
- Ability to manage multiple tasks efficiently under pressure.
