Certified Nurse Assistant/Home Health Aide- Enfield

We are seeking a skilled Certified Nurse Assistant/Home Health Aide to join our team in Enfield. As a key member of our home health care team, you will provide high-quality care to our patients in their homes, following physician orders and adhering to our policies and procedures.
Key Responsibilities- Develop and implement care plans in collaboration with our registered nurses.
- Provide personal care services, including bathing, grooming, and dressing, as well as assist with household tasks such as meal preparation and light housekeeping.
- Measure and record vital signs, including temperature, pulse, and blood pressure, as ordered by our medical team.
- Assist clients with self-administration of medication, as assigned by our registered nurses.
- Perform range of motion exercises and other simple procedures as an extension of therapy services.
- High school diploma or equivalent required.
- Completion of a federally compliant training program and successful completion of a competency evaluation program.
- One year of experience as an aide in an institution, home care, or hospice setting.
- Ability to work with minimal direct supervision and maintain a professional relationship with our clients and their families.
- Current driver's license and reliable transportation to and from work sites.
- Mileage reimbursement.
