We are seeking a skilled and compassionate Home Health Aide/Certified Nurse Assistant to join our team at Trinity Health. As a Home Health Aide, you will provide high-quality care to our patients in their homes, following physician orders and adhering to our policies and procedures.
Key Responsibilities- Assist patients with personal care activities, including bathing, dressing, and grooming
- Help patients with household tasks, such as meal preparation and light housekeeping
- Measure and record patients' vital signs, as ordered
- Perform range of motion and other simple procedures as assigned
- Assist patients with self-administration of medication, as assigned by an RN
- High School diploma or GED required
- Completion of federally compliant training program and successful completion of a competency evaluation program
- Documentation of one year's employment as an aide in an institution, home care, or hospice setting
- Ability to function in a practice environment with minimal direct supervision
- Current Driver's license and reliable transportation to and from work site
- Mileage reimbursement
- Medical, dental, and vision insurance - day one benefits
