Job Title: Certified Nurse Assistant/Home Health Aide

We are seeking a compassionate and skilled Certified Nurse Assistant/Home Health Aide to join our team at Trinity Health Of New England At Home. As a Certified Nurse Assistant/Home Health Aide, you will provide high-quality care to our patients in their homes, following physician orders and in compliance with state regulations.

Job Summary:

The Certified Nurse Assistant/Home Health Aide will provide paraprofessional services to our patients, including personal care activities, household tasks, and other duties as assigned. This is a full-time position, and we offer a competitive salary and benefits package.

Responsibilities:
  • Follows care plan developed by RN.
  • Reports client changes in condition to RN, as described on care plan.
  • Assists clients with personal care activities, including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
  • Assists clients with household tasks, including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
  • Measures and records clients' oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered.
  • Performs range of motion and other simple procedures as an extension of therapy services as assigned.
  • Assists clients in self-administration of medication, as assigned by an RN and documented in the clinical record.
Requirements:
  • High School diploma or GED required.
  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
  • Documentation of one year's employment as an aide in an institution, home care, or hospice setting.
  • Meets agency hiring criteria as defined in agency minimum standards.
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family.
  • Must accept responsibility for maintaining skills, learning, and adhering to agency policies and procedures on an ongoing basis.
  • Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
  • Must have a current driver's license and reliable transportation to and from work site.
