We are seeking a skilled Certified Nurse Assistant/Home Health Aide to join our team at Trinity Health Of New England At Home. As a Home Health Aide, you will provide high-quality care to our patients in their homes, following physician orders and adhering to our policies and procedures.
Responsibilities- Assist patients with personal care activities, including bathing, dressing, and grooming.
- Help patients with household tasks, such as meal preparation, light housekeeping, and laundry.
- Measure and record patients' vital signs, including temperature, pulse, and blood pressure.
- Perform range of motion exercises and other simple procedures as assigned.
- Assist patients with self-administration of medication, as assigned by a registered nurse.
- High school diploma or equivalent required.
- Completion of a federally compliant training program and successful completion of a competency evaluation program.
- One year of experience as an aide in an institution, home care, or hospice setting.
- Ability to work with minimal direct supervision and maintain a professional relationship with patients and their families.
- Current driver's license and reliable transportation.
- Mileage reimbursement.
