Certified Nurse Assistant/Home Health Aide

We are seeking a skilled Certified Nurse Assistant/Home Health Aide to join our team at Trinity Health Of New England At Home. As a member of our home health care team, you will provide exceptional care to our patients in the comfort of their own homes.
Key Responsibilities- Deliver high-quality patient care, following physician orders and adhering to our policies and procedures.
- Assist patients with daily living activities, including bathing, dressing, and grooming.
- Provide emotional support and companionship to patients and their families.
- Collaborate with our healthcare team to develop and implement personalized care plans.
- Document patient progress and report any changes or concerns to our team.
- High school diploma or equivalent required.
- Completion of a federally compliant training program and successful completion of a competency evaluation program.
- Minimum one year of experience as a home health aide or in a related field.
- Ability to work independently with minimal supervision and maintain a professional relationship with patients and their families.
- Current driver's license and reliable transportation.
- Mileage reimbursement.
