Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at Volunteers of America Mid-States. As a Case Manager, you will play a critical role in providing supportive services to veterans and their families who are experiencing homelessness or at-risk of homelessness.
Key Responsibilities- Provide outreach and engagement with community partners to increase awareness of our services and recruit landlords to work with our client families.
- Conduct comprehensive assessments and develop individualized case plans to address the unique needs of each participant.
- Link participants with appropriate agencies and service providers to access health care services, daily living services, and other supportive resources.
- Decide how resources are allocated to participants based on need and eligibility, according to grant guidelines.
- Provide education and support to participants on issues such as supportive services availability and participant rights.
- Assist participants in obtaining public and VA benefits, as well as other forms of assistance.
- Collaborate with other VOA Honor and Home Case Managers, Intake Coordinators, and Outreach Workers to ensure seamless service delivery.
- Bachelor's degree in social work or related field; at least five (5) years of experience in the field and no degree; or a Veteran with three (3) years of related work experience.
- Demonstrated ability to provide a reasonable combination of family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers.
- Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.
- Personal automobile, valid driver's license, liability insurance, and willingness to travel between counties up to 70% of the time.
