Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team. As a Home Care Registered Nurse, you will provide direct patient care in the home setting, working closely with patients, families, and healthcare teams to develop and implement personalized care plans. This is a PRN position, requiring a minimum of 48 hours of work every 6 weeks, including one weekend shift and one holiday per year.

Responsibilities
  • Provide direct nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current state licensure as a Registered Nurse (RN).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Valid Ohio driver's license and proof of automobile insurance.
  • Minimum one year of current clinical experience as a Registered Nurse, with a focus on home care or medical-surgical settings.
