Spiritual Support Counselor

POSITION SUMMARY:
This position is dedicated to addressing the spiritual requirements of hospice patients, their families, staff, and volunteers through direct engagement. It includes collaboration with local clergy and spiritual leaders.
KEY RESPONSIBILITIES:
SPIRITUAL EVALUATION AND ASSISTANCE
- Ensure the spiritual needs of patients and families are met and support the Interdisciplinary Team (IDT) with spiritual matters.
- Evaluate the spiritual needs of patients and families in relation to terminal illness.
- Conduct Initial Spiritual Evaluation and Care Plan for patients seeking counseling within 5 days of admission.
- Foster relationships with community religious representatives.
- Incorporate spiritual dimensions into care plans, assess their effectiveness, and provide follow-up as necessary.
- Educate team members and others on spiritual matters.
- Meet visitation expectations and address spiritual concerns through direct service or referrals.
- Contribute to patient care plans and offer support to families and caregivers.
- Facilitate religious rituals and memorial services as requested.
- Organize patient memorial services and assist with annual remembrance activities.
- Provide education on patient care to families and the community.
COLLABORATION WITH TEAM MEMBERS
- Communicate effectively with IDT members and patients/families to ensure continuity of care.
- Engage in IDT meetings and collaborate on patient care strategies.
- Recognize and respect the expertise of other team members.
- Provide training to staff as required.
- Maintain patient confidentiality in all communications.
DOCUMENTATION AND COMPLIANCE
- Ensure adherence to policies, regulations, and standards.
- Document patient interactions and evaluations.
- Complete assessments promptly.
- Participate in Quality Assurance initiatives.
- Maintain an updated Clergy Listing.
QUALIFICATIONS:
1. Completion of Clinical Pastoral Education training or equivalent experience.
2. Strong interpersonal and communication abilities.
3. Capacity to engage with individuals from diverse spiritual backgrounds.
4. Understanding of hospice care and terminal illness.
5. Proficiency in clinical assessment and care planning.
6. Familiarity with PC software is preferred.
7. Knowledge of regulations impacting clinical practice.
8. Background screening clearance is required.
PHYSICAL REQUIREMENTS:
1. Ability to effectively communicate and fulfill job responsibilities.
2. Capability to lift up to 15 pounds.
3. Maintain a personal vehicle and valid driver's license.
4. Flexibility for occasional evening or weekend responsibilities.
