Job Description

Ranked nationally two years in a row as a "Best Places to Work" by Modern Healthcare, YoloCares is seeking an experienced and innovative RN-Case Manager who is squarely focused on collaboration, relationship management, and education.

We are looking for a full-time dedicated Case Manager - Registered Nurse to provide skilled nursing care.

Key Responsibilities:
  • Completes the nursing assessment of assigned patients and maintains an ongoing assessment within hospice guidelines to determine if patient fulfills hospice criteria.
  • Collaborates with other core disciplines (medicine, social work, and spiritual care counseling) in accordance with the Plan of Care to meet care goals.
  • Coordinates provision of necessary medications, durable medical equipment, and disposable medical supplies, ensuring cost-effective and timely provision.
  • Provides ongoing professional nursing care for each assigned patient, regularly assessing and reevaluating patient nursing needs and related patient/family support needs.
  • Communicates with the Attending Physician or designee regarding changes in condition and emergent patient needs, obtaining and documenting necessary physician orders.
Requirements:
  • A current license in the State of California as a Registered Nurse.
  • Ability to communicate effectively verbally and in writing in English is required; Spanish language skills are also encouraged.
  • Two (2) years of recent clinical nursing experience is preferred, with experience in home care or hospice care preferred.
  • Must demonstrate strong clinical nursing skills, an understanding of, and commitment to, the general philosophy and goals of hospice care.
