**Position at Vassar Brothers Medical Center**

**Summary:**

Under the supervision of the Nurse Manager/Program Director, the Wound Care Program Coordinator collaborates effectively with physicians, fellow Wound Care Center staff, and other healthcare professionals to ensure high-quality care for patients with wounds. Operating with a significant level of independence, the Coordinator is responsible for managing assigned patients, overseeing their care from planning through implementation and evaluation.

**Responsibilities:**

  1. Deliver patient care that is continuously assessed, planned, and adjusted as necessary.
  2. Design patient care plans that align with realistic, measurable goals consistent with the medical treatment plan, including discharge planning.
  3. Organize and prioritize tasks to provide health information, discharge planning, and patient care in a secure environment, enhancing health outcomes throughout the continuum of care.
  4. Exhibit professionalism as a nurse and case manager through precise, clear, and timely communication, both written and verbal.
  5. Demonstrate knowledge and professional application of current trends and practices within the healthcare sector.
  6. Utilize materials, equipment, and time in a safe, therapeutic, and cost-effective manner.
  7. Engage in Quality Assurance initiatives.
  8. **Guest Relations:** Uphold the dignity and respect of all patients, family members, visitors, and medical center staff, in line with the philosophy of Vassar Brothers Medical Center.
  9. Embody and promote Nuvance Health Foundational Values (Personal, Imaginative, Agile, Connected).
  10. Perform additional duties as necessary.
**Other Information:**
  1. A Bachelor’s degree in nursing (BSN) is required, with graduation from an accredited School of Nursing.
  • Wound Care Certification is preferred.
Three (3) years of experience as a staff nurse in a medical/surgical or home care environment is preferred.
  • Experience in Wound Care is preferred.
Possession of a current NYS RN license is required. **Working Conditions:**

**Manual:** Significant manual skills/motor coordination and finger dexterity are necessary.

**Occupational:** There is a significant occupational risk involved.

**Physical Effort:** The role requires medium to heavy physical effort, with the potential to exert up to 35 lbs. of force.

**Physical Environment:** There may be some exposure to dirt, odors, noise, human waste, etc.
