Job Overview

Position Summary:

Under the supervision of the Nurse Manager/Program Director, the Wound Care Coordinator plays a vital role in the health care team by effectively communicating and collaborating with physicians and Wound Care Center staff to ensure high-quality care for patients with wounds. This position requires a high level of autonomy, with the Coordinator taking on the responsibility and accountability for managing assigned patients, planning, implementing, and evaluating their care.

Key Responsibilities:

  1. Deliver patient care that is consistently assessed, planned, and adjusted as necessary.
  2. Develop patient care plans that are realistic, measurable, and aligned with the medical plan of care, including discharge planning.
  3. Organize and prioritize tasks to provide health information, discharge planning, and patient care in a safe environment, ensuring optimal health outcomes throughout the continuum of care.
  4. Exhibit professionalism as a nurse and case manager through accurate, clear, concise, and timely communication, both written and verbal.
  5. Stay informed about current trends and practices in the health care field to enhance professional knowledge.
  6. Utilize materials, equipment, and time in a safe, therapeutic, and cost-effective manner.
  7. Engage in Quality Assurance initiatives.
  8. Demonstrate respect and dignity towards all patients, family members, visitors, and medical center personnel, in line with the philosophy of Nuvance Health.
  9. Uphold and exemplify Nuvance Health's foundational values: Personal, Imaginative, Agile, Connected.
  10. Perform additional duties as required.

Additional Information:

  1. A Bachelor’s degree in nursing (BSN) is required, with graduation from an accredited School of Nursing.
  • Wound Care Certification is preferred.
  • A minimum of three (3) years of experience as a staff nurse in a medical/surgical or home care setting is preferred.
    • Experience in Wound Care is preferred.
  • Must possess a current NYS RN license.

Working Conditions:

Manual: Significant manual skills/motor coordination and finger dexterity required.

Occupational: Significant occupational risk present.

Physical Effort: Medium to heavy effort required; may need to exert up to 35 lbs. of force.

Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
