ROLE SUMMARY: The Wound Ostomy Continence Nurse (WOCN) is a licensed nursing professional who possesses advanced knowledge and skills in the management of wounds and ostomies. This role involves serving as a clinical expert and resource for nursing staff and physicians, focusing on the assessment and development of care plans for patients experiencing specific conditions related to the gastrointestinal, genitourinary, and integumentary systems. Key areas of focus include ostomies, fistulas, drainage tubes, dermal wounds, and incontinence. The WOCN fulfills multiple roles, including clinician, consultant, educator, and researcher.
QUALIFICATIONS
Education: Bachelor’s degree required; Master’s degree preferred.
Certification: CWOCN certification is mandatory.
Experience: A minimum of 5 years of relevant nursing experience is required.
Licensure: Must hold a valid New York State Registered Nurse license.
Key Responsibilities
1. Clinical Responsibilities:
1.1 Employ the nursing process to assess, plan, and implement care strategies, guiding nursing staff in the management of patients with ostomies, wounds, and incontinence issues.
1.2 Collaborate with physicians and nursing teams in stoma siting and care.
1.3 Initiate and instruct on appropriate stoma care procedures and skin care techniques for patients and staff.
1.4 Monitor stoma characteristics and alert physicians to any complications.
1.5 Measure and fit stomas with appropriate appliances, ensuring patients receive suitable equipment.
1.6 Develop discharge plans and provide clinical support for patients transitioning to community care.
1.7 Coordinate referrals to other healthcare services as needed.
1.8 Administer debridement of non-viable tissue under physician orders.
1.9 Assist in surgical procedures as required.
2. Consultative Role:
2.1 Offer guidance to staff in creating and executing protocols for managing skin integrity issues.
2.2 Recommend adjustments to wound care orders based on patient assessments.
2.3 Educate staff on the identification and management of skin breakdown factors.
2.4 Evaluate patient responses to treatments and adjust care plans accordingly.
2.5 Act as a liaison between patients and healthcare providers.
2.6 Provide consultative services for patients with continence disorders.
3. Educational Initiatives:
3.1 Conduct training sessions for new nursing and therapy staff on wound care and ostomy management.
3.2 Facilitate ongoing education to ensure staff remain knowledgeable in wound and ostomy care.
3.3 Assess educational needs of patients and tailor instruction accordingly.
3.4 Educate patients and their families on stoma care and appliance management.
3.5 Utilize effective teaching materials and document patient outcomes.
3.6 Provide necessary instruction on bowel and bladder control concepts.
3.7 Design and implement educational programs for targeted audiences.
3.8 Lead seminars and workshops on relevant topics.
4. Research and Development:
4.1 Maintain records and statistics for reporting purposes.
4.2 Recommend standards for cost-effective supplies and equipment for wound and ostomy care.
4.3 Engage in relevant research projects.
4.4 Conduct and evaluate prevalence studies.
4.5 Participate in quality improvement initiatives.
5. Professional Growth:
5.1 Actively engage in local and national professional organizations.
5.2 Create a personal development plan to enhance professional skills.
5.3 Attend continuing education programs related to wound and skin care.
5.4 Participate in agency-wide committees and projects.
5.5 Contribute to professional publications and support relevant organizations.
5.6 Maintain CWOCN certification.
6. Program Management:
6.1 Establish and facilitate ostomy support groups.
6.2 Lead a dedicated wound care team.
6.3 Recommend referrals to specialized care centers.
6.4 Consult with home health and hospice staff on patient needs.
6.5 Promote wound and ostomy care services.
6.6 Develop and implement comprehensive wound care protocols.
