Position Summary:
Under the guidance of the Nurse Manager/Program Director, the Wound Care Coordinator plays a pivotal role in ensuring high-quality care for patients with wounds. This position requires effective communication and collaboration with physicians, WCC staff, and other healthcare professionals, fostering a cohesive approach to patient care. The Coordinator operates with a significant degree of independence, taking on the responsibility and accountability for managing assigned patients throughout their care journey.
Key Responsibilities:
- Deliver patient care that is continuously assessed, planned, and adjusted as necessary.
- Develop patient care plans that align with realistic, measurable goals and adhere to the medical plan of care, including discharge strategies.
- Organize and prioritize tasks to provide health information, discharge planning, and patient care in a secure environment, enhancing overall health outcomes.
- Exhibit professionalism as a nurse and case manager through precise, clear, and timely written and verbal communication.
- Stay informed and utilize current trends and practices within the healthcare sector effectively.
- Employ resources, equipment, and time in a safe, therapeutic, and cost-efficient manner.
- Engage in Quality Assurance initiatives.
- Demonstrate respect and dignity towards all patients, family members, visitors, and medical personnel, in line with the philosophy of Nuvance Health.

- Perform additional duties as necessary.
Additional Information:
- A Bachelor’s degree in nursing (BSN) is required, with graduation from an accredited School of Nursing.
- Wound Care Certification is preferred.
- Three (3) years of experience as a staff nurse in a medical/surgical or home care environment is preferred.
- Experience in Wound Care is preferred.
- Current NYS RN license is required.
Working Conditions:
Manual: Significant manual skills/motor coordination and finger dexterity required.
Occupational: Notable occupational risks involved.
Physical Effort: Medium to heavy physical effort; may require exerting up to 35 lbs. of force.
