Job Overview

Position Summary:

Under the guidance of the Nurse Manager/Program Director, the Wound Care Coordinator plays a pivotal role in ensuring exceptional care for patients with wound-related issues. This position requires effective communication and collaboration with physicians, Wound Care Center staff, and other healthcare professionals to deliver consistent and high-quality patient care. The Coordinator operates with a significant degree of independence, taking on the responsibility of case management for assigned patients, including planning, implementing, and evaluating their care.

Key Responsibilities:

  1. Continuously assess, plan, and modify patient care to meet evolving needs.
  2. Develop care plans that align with realistic and measurable goals, ensuring they correspond with the overall medical treatment strategy for each patient, including discharge planning.
  3. Effectively organize and prioritize tasks to provide health information, discharge planning, and patient care in a secure environment, aiming to enhance patient health throughout their care journey.
  4. Exhibit professionalism as a nurse and case manager through precise, clear, and timely written and verbal communication.
  5. Stay informed and apply current trends and practices within the healthcare sector.
  6. Utilize resources, equipment, and time in a safe, therapeutic, and cost-efficient manner.
  7. Engage in Quality Assurance initiatives.
  8. Demonstrate respect and dignity towards all patients, family members, visitors, and healthcare personnel, in line with the philosophy of Nuvance Health.
  9. Embody and promote Nuvance Health's core values (Personal, Imaginative, Agile, Connected).
  10. Perform additional duties as required.

Additional Information:

  1. A Bachelor’s degree in nursing (BSN) is required, along with graduation from an accredited School of Nursing.
  • Wound Care Certification is preferred.
  • A minimum of three (3) years of experience as a staff nurse in a medical/surgical or home care environment is preferred.
    • Experience in wound care is advantageous.
  • Possession of a current NYS RN license is mandatory.

Working Conditions:

Manual: Requires significant manual skills and dexterity.

Occupational: Involves considerable occupational risk.

Physical Effort: Medium to heavy physical effort may be required, including the ability to exert up to 35 lbs. of force.

Physical Environment: May encounter exposure to dirt, odors, noise, and other environmental factors.
