Responsabilidades Esenciales:
- Ejecutar todas las funciones y responsabilidades de un Coordinador de Atención, incluyendo la gestión de una carga de casos modificada.
- Supervisar un equipo de 8-10 Coordinadores de Atención.
- Revisar la presentación de toda la documentación en los registros de los clientes (incluyendo notas de progreso, evaluaciones de ingreso, actualizaciones anuales y documentos de alta) para todos los miembros del equipo, incluyendo la puntualidad de la presentación según los plazos establecidos, así como el contenido y la calidad de los servicios proporcionados.
- Mantener un conocimiento operativo de las cargas de casos del equipo.
- Capacitar a los nuevos miembros del personal en todas las áreas de la política del programa, requisitos/plazos del hogar de salud y presentación de documentos en el EMR.
- Asistir a visitas de ingreso/visitas domiciliarias con los miembros del equipo según sea necesario para garantizar la seguridad del personal, proporcionar orientación a los nuevos empleados o ofrecer consulta según se solicite.
- Actuar como consultor para todos los miembros del equipo para proporcionar información sobre recursos comunitarios, programas de bienestar social y otra información según sea necesario.
- Abordar las preocupaciones y/o quejas de los clientes que no pueden resolverse directamente con el Coordinador de Atención.
- Co-facilitar reuniones semanales del equipo con el coordinador del programa para revisar preocupaciones/asuntos con los clientes y transmitir información sobre cambios/actualizaciones del programa, así como compartir cualquier nueva información de recursos entre el equipo.
- Asistir a reuniones operativas quincenales con el Director del Programa para discutir problemas/preocupaciones del programa, cambios de política y transmitir cualquier inquietud del personal.
- Asistir a supervisiones individuales según lo programado con el Director del Programa para discutir necesidades e inquietudes y trabajar en el desarrollo de habilidades de supervisión.
- Asistir a capacitaciones periódicas para mejorar los niveles de habilidad y mantenerse al tanto de los cambios en el sistema (incluyendo MAPP, HARP y DSRIP).
- Participar en actividades a través del hogar de salud y otras fuentes para mejorar el desarrollo del programa y ayudar en el desarrollo y mejora de los sistemas de entrega de servicios de coordinación de atención.
- Trabajar en estrecha colaboración con los Coordinadores de Programa y el Director para abordar cualquier problema que surja con el personal y garantizar la implementación fluida de la política del programa.
- Asistir al Coordinador del Programa con cualquier tarea adicional relacionada con las necesidades del departamento.
- Título de maestría en el campo de servicios sociales más dos años de experiencia en Servicios Humanos o un título de licenciatura en el campo de servicios sociales más cuatro años de experiencia en Servicios Humanos preferido. Se requiere un título de maestría en el campo de servicios sociales más un año de experiencia en Servicios Humanos o un título de licenciatura en cualquier campo más cinco años de experiencia en Servicios Humanos.
- Licencia de conducir de NY y acceso a un vehículo.
