Hospice RN-Care Specialist

Job Summary:
We are seeking a compassionate and skilled Hospice RN-Care Specialist to join our team. As a Hospice RN-Care Specialist, you will provide high-quality care to patients and families in local nursing homes in Schenectady County.
Responsibilities:
- Case manage patients in the nursing home setting
- Respond to symptom management needs of hospice patients
- Coordinate with the hospice team to provide added support to residents and families
- Work collaboratively with nursing home staff to ensure high-quality care
Requirements:
- Current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1 year experience as an RN required, 1-2 years long-term care experience preferred
- Previous hospice experience preferred but not required
- Valid Driver's License and Reliable vehicle
