Responsibilities- Provide patients with diagnosis, treatment, and care under the supervision of a physician
- Focus on providing palliative care to patients and their families
- Assist families along their health care journeys with love and compassion
- Participate in interdisciplinary team meetings to develop and implement patient care plans
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
- 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
- Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
