Job Title: Medication Safety Pharmacist

About the Role:

The Medication Safety Pharmacist will play a critical role in ensuring the safe and effective use of medications throughout the pharmacy department and hospital. This position requires a strong clinical background and excellent communication skills to effectively collaborate with various departments and stakeholders.

Key Responsibilities:

  • Develop and implement medication safety initiatives to reduce errors and improve patient outcomes.
  • Conduct regular safety assessments and analyze medication error/adverse drug event data to identify areas for improvement.
  • Collaborate with the Medication Safety Committee and Pharmacy & Therapeutics Committee to develop and implement policies and procedures related to medication safety.
  • Provide education and training to healthcare staff on medication safety best practices.
  • Monitor and report on medication safety metrics to ensure compliance with regulatory requirements.

Requirements:

  • Graduation from an accredited school of pharmacy.
  • Three years of hospital pharmacy experience.
  • Experience with Epic system and medication builds, order panel and order set builds.
  • Demonstrated leadership ability.
  • Licensed as a Pharmacist by the Virginia Department of Health Professions.

Preferred Qualifications:

  • Epic Willow and/or Beacon analyst certification.
  • Medication Safety Certification within 12 months.
