Job Description

This full-time position requires a highly motivated and empathetic Social Worker to join our multidisciplinary team. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  1. Develop and implement patient care plans, incorporating input from medical staff, family members, and other stakeholders
  2. Provide emotional support and counseling to patients and their loved ones, addressing their concerns and fears about end-of-life care
  3. Facilitate access to community resources and benefits, such as Medicare, Medicaid, and private insurance
  4. Coordinate with medical teams to ensure smooth transitions between levels of care
  5. Document patient interactions and outcomes accurately and efficiently

Requirements:

  • Bachelor's or Master's degree in Social Work (MSW)
  • Licensure from the state Board of Social Work Examiners
  • At least 1 year of post-graduate experience in hospice or palliative care
  • Valid driver's license
