Hospice Care Partners is seeking a dedicated and compassionate Social Worker to join our team in providing exceptional care to terminally ill patients and their families. Job OverviewWe are committed to delivering high-quality, patient-centered care that addresses the physical, emotional, and spiritual needs of our patients. As a Hospice Social Worker, you will play a vital role in coordinating with our medical team to develop and implement personalized plans of care.
Key Responsibilities:
- Conduct assessments of patients' psychological responses to terminal illness and provide supportive counseling and teaching
- Evaluate patients' financial situations and assist them in accessing benefits through Medicare, Medicaid, and private insurance
- Consult with patients and families on funeral arrangements
- Maintain accurate records of patient interactions and outcomes
- Participate in interdisciplinary team meetings to ensure comprehensive care planning
Requirements:
- Bachelor's or Master's degree in Social Work (MSW)
- Licensure from the state Board of Social Work Examiners
- At least 1 year of post-graduate experience in hospice or palliative care
- Valid driver's license
