About the Role:

We are seeking a highly motivated individual with a Master's degree in social work, state social work license, and at least 1 year of post-master's degree experience. The successful candidate will coordinate with medical teams to develop and implement plans of care, assess patient/family responses to terminal illness, and provide supportive counseling and teaching.

Key Responsibilities:

  • Develop and implement individualized plans of care in collaboration with healthcare teams.">
  • Evaluate patients' eligibility for hospice services and make referrals as necessary.">
  • Provide emotional support and guidance to patients and their families through various stages of the illness.">
  • Assist patients with understanding and accessing benefits through Medicare, Medicaid, and private insurance.">
  • Maintain accurate records and documentation of patient interactions and progress.">

Requirements:

  • Master's degree in social work from an accredited institution.">
  • State social work license in good standing.">

  • Excellent communication, interpersonal, and organizational skills.">
  • Valid state driver's license and reliable transportation.">

Salary Range: $70,000 - $90,000 per annum, depending on qualifications and experience. Benefits include a competitive salary, comprehensive health insurance, retirement plan, and paid time off.
