Job Summary:
We are seeking an experienced Registered Nurse with a passion for palliative care to join our interdisciplinary team. As a Hospice Palliative Care Nurse Specialist, you will be responsible for administering nursing care to patients of all ages in their place of residence, coordinating care with the patient, family, and referring agency, and assuming responsibility for coordination of care.

About You:
To be successful in this role, you will need to possess excellent communication skills, assessment skills, and strong clinical documentation skills. You should have a good understanding of the principles of palliative care and be able to work collaboratively as part of a team. A minimum of 2 years of experience working in a Hospice agency is preferred, as is Home Care Home Base experience. You will need to be able to read, write, and speak the English language fluently, and have reliable transportation with valid and current auto liability insurance.

The Role:
As a Hospice Palliative Care Nurse Specialist, your key responsibilities will include:

  • Conducting initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs;
  • Applying specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility;
  • Implementing and developing the plan of care to ensure quality and continuity of care and recommending revisions as necessary;
  • Consulting with and educating the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs;
  • Determining the scope and frequency of services needed based on acuity and patient/family needs; and
  • Monitoring assigned cases to ensure compliance with requirements of third-party payors.
