Brockton Area Multi-Services is seeking Community Support Specialists for various programs across Massachusetts. This role is ideal for individuals pursuing careers in healthcare and social services. We are expanding our team of committed human service and healthcare professionals who assist individuals with developmental disabilities, intellectual challenges, acquired brain injuries, and complex medical needs in their residences. Community Support Specialists will collaborate with nursing staff in a small home setting, delivering medical assistance and aiding with daily living activities. Our specialists foster a compassionate, secure, and engaging home atmosphere for those we serve.

Key Responsibilities:

  1. Assist individuals with daily living tasks, including cooking, cleaning, general upkeep, shopping, and financial management.
  2. Provide instruction in social skills and daily living activities, such as household maintenance, self-care, hygiene, budgeting, transportation, and community engagement.
  3. Support the creation and execution of Program Specific Treatment Plans (PSTPs), including behavioral objectives.
  4. Participate in Individual Support Plan (ISP) meetings and other required reviews.
  5. Adhere to the Department of Public Health's Medication Administration Program (MAP) policies, including training, medication handling, and documentation.
  6. Maintain accurate records and documentation of daily progress, staff logs, and other required reports.
  7. Transport individuals to and from various appointments and activities, ensuring compliance with driving policies.
  8. Encourage leisure activities and promote independence in choosing and participating in these activities.
  9. Design and implement group and individual activities that foster community integration.
  10. Manage case responsibilities as assigned, including medical and legal matters.
  11. Ensure the safety of clients and adhere to program staffing standards.
  12. Act as an advocate and role model for consumers.
  13. Provide a supportive environment that empowers individuals to make choices regarding their daily routines.

Communication and Teamwork:

  1. Follow protocols for reporting any suspected abuse or neglect.
  2. Document data at the end of each shift, including financial audits and medication records.
  3. Facilitate fire drills as per the program's safety plan.
  4. Stay informed about ongoing program developments and communicate any concerns to supervisors.
  5. Maintain professionalism and respect in all communications.
  6. Assist in training new employees as needed.
  7. Participate in monthly staff meetings and adhere to all organizational policies.
  8. Understand and apply safety precautions in all relevant situations.
  9. Complete additional duties as assigned.

Qualifications:

  • High school diploma or equivalent; Associate's or Bachelor's degree preferred.
  • Male candidates are preferred for this position due to specific operational requirements.
  • A valid driver's license and a clean driving record are required.
  • MAP certification must be obtained within six months of hire.

Experience:

  • Experience in behavior management and de-escalation techniques is essential.
  • Prior experience working with individuals with developmental or physical disabilities is preferred; mental health experience is highly desirable.

Skills and Abilities:

  • Proficient in using computers for data entry and communication.
  • Ability to understand materials related to medication administration and individual support plans.
  • Strong written and verbal communication skills in English.
  • Physical capability to lift and assist individuals as needed.
