About the Role

We are seeking a dedicated and compassionate Support Specialist to join our dynamic Day Center team at Community Connections, Inc. As a Support Specialist, you will play a vital role in supporting individuals with disabilities, helping them thrive and reach their full potential.

Key Responsibilities
  • Assist individuals in developing and refining skills, promoting functional independence during breaks, mealtimes, and other specific routines.
  • Help to implement individualized goals within a group setting or one-to-one situation.
  • Assist individuals physically as necessary, ensuring their health, safety, and general well-being.
  • Document individuals' progress toward goals and objectives.
  • Ensure and safeguard the human dignity, rights, and liberties of the individuals we serve.
Requirements
  • High School Diploma or General Education Degree (GED)
  • Experience and/or training working with individuals with disabilities preferred
  • Valid driver's license and satisfactory driving record preferred
  • Ability to lift 30lbs, assist individuals in/out of wheelchairs or up from/down to mats
  • Demonstrated ability to work independently
  • Bi-lingual preferred (Portuguese)
