About the Role

We are seeking a skilled Social Service Specialist II to join our team at the Department of Health and Social Services (DHSS). As a key member of our team, you will play a vital role in improving the quality of life of Delaware citizens by promoting health and well-being, fostering self-sufficiency, and protecting vulnerable populations.

Key Responsibilities

  • Interview clients and conduct financial assessments to identify their needs and develop personalized plans for support.
  • Review cases for referrals and ensure that clients receive the necessary services and resources.
  • Document data and maintain accurate records to track client progress and outcomes.
  • Assist clients with form completion and ensure they meet their responsibilities.
  • Refer clients to appropriate agencies and resources, and coordinate services to meet their needs.
  • Establish strong relationships and build trust with clients to ensure successful outcomes.

Requirements

  • 6 months of experience in health or human services support, including interviewing clients and assessing personal, health, social, or financial needs.
  • 6 months of experience in record keeping and data entry.
  • 6 months of experience in applying laws, rules, regulations, standards, policies, and procedures.
  • 6 months of experience in using an automated information system to enter, update, modify, delete, retrieve, inquire, and report on data.
