About the Role

We are seeking a highly skilled and compassionate Social Service Specialist II to join our team at the Department of Health and Social Services (DHSS). As a key member of our team, you will play a vital role in improving the quality of life of Delaware citizens by promoting health and well-being, fostering self-sufficiency, and protecting vulnerable populations.

Key Responsibilities

  • Interview clients and families to assess their needs and develop personalized plans to support their well-being.
  • Conduct financial assessments and review cases to identify referrals and provide guidance on available resources.
  • Document data and maintain accurate records to ensure effective case management and reporting.
  • Assist clients with form completion, ensure they meet their responsibilities, and refer them to appropriate agencies.
  • Coordinate services, schedule appointments, and respond to inquiries from agencies, clients, and courts.
  • Conduct outreach, perform on-site visits, and deliver educational sessions to promote health and well-being.
  • Perform data entry, maintain case records, prepare reports, and follow up to ensure recommendations are completed.

Requirements

  • 6 months of experience in health or human services support, including interviewing clients and assessing personal, health, social, or financial needs.
  • 6 months of experience in record keeping.
  • 6 months of experience in applying laws, rules, regulations, standards, policies, and procedures.
  • 6 months of experience in using an automated information system to enter, update, modify, delete, retrieve/inquire, and report on data.
