Job Title: Rehabilitation Specialist II

Community Connections is seeking a highly skilled Rehabilitation Specialist II to join our Assertive Community Treatment (ACT) team. As a key member of our clinical team, you will work directly with consumers to deliver ACT services and support their recovery journey.

Key Responsibilities:
  • Develop and implement individualized recovery plans in collaboration with the clinical team and consumers.
  • Provide direct support and therapy to consumers, focusing on skill development, symptom management, and relapse prevention.
  • Participate in crisis prevention and intervention efforts, ensuring the safety and well-being of consumers.
  • Collaborate with the clinical team to develop and lead psycho-educational and skill development groups.
  • Conduct diagnostic assessments and treatment plans under the supervision of the clinical manager.
Requirements:
  • Masters' degree in social work, psychology, or a relevant human services discipline.
  • Active LGSW or LCPC license issued in the District of Columbia.
  • Proficient in Microsoft Office products (Word, Excel, Outlook).
  • Case management experience and documentation skills preferred.
