Job Overview

POSITION SUMMARY:

The Forensic Care Coordinator will deliver advocacy, organization, and ongoing evidence-based support for individuals and families requiring assistance through a collaborative approach involving triage, evaluation, planning, and communication. This role is part of a multidisciplinary treatment team and reports to the unit or department supervisor. The coordinator is responsible for ensuring all legal documents are current and maintained in client files, attending legal proceedings, collaborating with legal representatives, and facilitating competency restoration training.

QUALIFICATIONS:

A Bachelor's degree in a Human Services-related field is required, along with a minimum of one year of experience in the mental health sector. Candidates must possess the ability to read, analyze, and interpret clinical documentation, procedures, or governmental regulations. Proficiency in writing reports and documenting clinical activities is essential, as is the ability to communicate effectively with managers, patients, service providers, managed care organizations, family members, and other stakeholders.

A valid Florida Driver's license, reliable personal transportation, and valid vehicle insurance are mandatory.

FUNCTIONAL RESPONSIBILITIES INCLUDE:

  • Conducting comprehensive assessments and coordinating community resources tailored to patient needs in accordance with program requirements.
  • Completing all clinical and administrative documentation within established timelines as per program guidelines.
  • Maintaining effective communication and fostering cooperative relationships with both internal and external resources.
  • Developing service plans, treatment plans, and/or aftercare plans with clearly defined goals and objectives in line with program standards.
  • Ensuring efficient workflow management and organization in accordance with program requirements.
  • Upholding high-quality standards for clinical documentation as mandated by program protocols.
  • Performing additional duties as assigned or required by program specifications.

COMPLIANCE RESPONSIBILITIES INCLUDE:

  • Adhering to all relevant federal, state, local, and company-maintained standards of compliance, ethics, and policies related to the administration and delivery of agency services.
  • Meeting deadlines, requirements, and expectations set forth by the supervisor, agency, and department.
  • Completing administrative tasks and data entry accurately and in a timely manner.

CUSTOMER SERVICE RESPONSIBILITIES INCLUDE:

  • Delivering exceptional customer service in all communications and interactions with staff and the general public.
