Work Schedule: Day shift, 40 hours per week | No Supervisory Duties | No Travel Required | Compensation based on experience

Position Summary:

The primary objective of our nursing staff is to ensure an exceptional patient experience while delivering professional nursing services across various environments. The Oncology Nurse Navigator collaborates with healthcare providers and a multidisciplinary team to offer both physical and emotional support to patients and their families. This role involves assessing, planning, implementing, and evaluating oncology health services. It includes identifying high-risk patients and developing educational plans tailored to their needs, fostering effective communication among patients, families, and nursing staff.

Essential Qualifications:

Education:

  • Bachelor of Science in Nursing (BSN) is required.

Experience:

  • A minimum of 5 years of oncology nursing experience is preferred.
  • Comprehensive knowledge of clinical practices is essential.

Licenses/Certifications:

  • Must hold a valid Registered Nurse (RN) license in North Carolina.
  • Current Basic Life Support (BLS) certification is required.
  • Advanced Cardiovascular Life Support (ACLS) certification must be obtained within six months of hire.
  • Oncology Certified Nurse certification is preferred.
  • ONS Chemotherapy/Biotherapy certification is mandatory.

Key Responsibilities:

  1. Engage in the assessment, planning, implementation, and evaluation of navigation services.
  2. Follow up on team recommendations and assist in care coordination to ensure timely follow-up and treatment.
  3. Oversee patient care coordination and develop strategies to address actual or potential barriers to care.
  4. Communicate patient status and needs effectively to the healthcare team.
  5. Collaborate with other care providers to enhance patient outcomes.
  6. Assist patients in transitioning from active treatment to survivorship or end-of-life care.
  7. Facilitate timely scheduling of appointments and diagnostic tests to streamline the care process.
  8. Provide psychosocial support and facilitate appropriate referrals during emotionally challenging times.
  9. Deliver individualized education regarding diagnosis, treatment options, and post-treatment care.
  10. Ensure that discharge instructions are clearly understood by patients and their families.
  11. Participate in outreach initiatives to promote community health.
  12. Demonstrate a commitment to quality care and continuous improvement.
  13. Utilize critical thinking skills to achieve optimal patient outcomes.
  14. Maintain current knowledge and skills in oncology care.
  15. Perform direct patient care activities as necessary.
  16. Adhere to all regulatory requirements and maintain confidentiality.
  17. Support the hospital's mission and promote a positive work environment.
