KEY RESPONSIBILITIES:
Core Duties:
As a registered nurse, you will:
- Conduct initial evaluations and ongoing assessments of patients' nursing needs.
- Develop and update the hospice care plan in collaboration with patients, families, physicians, and the Interdisciplinary Group (IDG).
- Provide specialized nursing services and initiate preventive care procedures.
- Maintain thorough clinical documentation and progress notes.
- Coordinate care services and communicate changes in patient conditions to the healthcare team.
- Advocate for patients and families regarding end-of-life care decisions.
- Participate in community coverage, including nursing homes and hospitals, as required.
- Engage in on-call duties during evenings, weekends, and holidays as necessary.
QUALIFICATIONS:
- Must be a graduate of an accredited nursing program and hold a valid New York State Registered Nurse (RN) license.
- A minimum of one year of clinical nursing experience in a hospital or home care setting is required; or a BSN with at least six months of relevant experience.
- Strong communication skills, both verbal and written, along with good organizational and computer proficiency.
- Availability for occasional evening and weekend hours in clients' homes is necessary.
- A current NYS driver's license and reliable transportation to client locations are required.

Salary Range:
$73,600 to $86,500
