Position Details:

Full-time, Day shift in the Oncology department, working 40 hours per week with no supervisory responsibilities and no travel required.

POSITION SUMMARY:

The primary responsibility of the Oncology Nurse Navigator is to ensure an exceptional patient experience while delivering professional nursing services across diverse environments. This role involves collaborating with physicians and a multidisciplinary team to provide both physical and emotional support to patients and their families. The Oncology Nurse Navigator plays a crucial role in assessing, planning, implementing, and evaluating oncology health services, identifying high-risk patients, and developing tailored education plans based on individual needs.

Key Responsibilities:

  1. Engage in the assessment, planning, implementation, and evaluation of navigation services.
  2. Coordinate care and follow up on team recommendations to ensure timely treatment.
  3. Oversee patient care coordination and address barriers to care.
  4. Facilitate referrals to specialty services and assist with prior authorizations.
  5. Communicate patient status and needs effectively to the care team.
  6. Collaborate with other healthcare providers to optimize patient outcomes.
  7. Support patients transitioning from active treatment to survivorship or end-of-life care.
  8. Ensure timely scheduling of appointments and diagnostic tests to maintain continuity of care.
  9. Provide psychosocial support and facilitate appropriate referrals during challenging times.
  10. Deliver individualized education regarding diagnosis, treatment options, and post-treatment care.
  11. Assist patients and families in understanding discharge instructions.
  12. Offer guidance and education to help patients cope with their cancer diagnosis.
  13. Participate in community outreach initiatives.
  14. Demonstrate a commitment to quality and patient-centered care.
  15. Utilize critical thinking to achieve optimal patient outcomes.
  16. Maintain current knowledge and skills in oncology nursing.
  17. Perform direct patient care activities as necessary.
  18. Adhere to all regulatory requirements and maintain confidentiality.

Minimum Qualifications:

Education:

  • Bachelor of Science in Nursing (BSN) required.

Experience:

  • Preferred 5 years of oncology nursing experience.
  • Knowledge of clinical practices is essential.

Licenses/Certifications:

  • Must hold a valid Registered Nurse (RN) license in the state of NC.
  • Current Basic Life Support (BLS) certification required.
  • Advanced Cardiovascular Life Support (ACLS) certification must be obtained within 6 months of hire.
  • Oncology Certified Nurse certification is preferred.
  • ONS Chemotherapy/Biotherapy certification is mandatory.
