Position Overview

Hometown Health Center is seeking a dedicated Patient Referral Specialist to enhance our team. In this role, you will oversee and facilitate external specialty appointments, ensuring a smooth continuum of patient care. Become a vital part of our healthcare team, contributing to the delivery of essential medical services and making a significant difference in the lives of our patients.

KEY RESPONSIBILITIES

  • Oversee and manage all medical referrals for the healthcare professionals at Hometown Health Center utilizing the Electronic Health Record (EHR) system.
  • Engage directly with patients to foster relationships and share pertinent information.
  • Employ the EHR system in accordance with departmental guidelines for precise data entry and information retrieval.
  • Handle insurance referrals and maintain comprehensive records for each authorization.
  • Collaborate closely with major insurance providers to ensure timely completion of referrals and authorizations.
  • Serve as a communication bridge between contracted referral services.

QUALIFICATIONS

  • At least two years of experience in a medical office setting, with prior experience in referrals.
  • Proficient knowledge of medical billing and coding processes.
  • Excellent organizational and communication skills.
  • Ability to work both independently and as part of a team.
  • Strong comprehension and articulation of written and verbal communications.
  • Willingness to learn, adapt, and embrace new challenges.
  • Compliance with Hometown Health Center's employee immunization policies.

EDUCATIONAL BACKGROUND

  • High School diploma or equivalent is required.
  • Demonstrated understanding of insurance referral protocols.
  • Basic knowledge of medical terminology, tests, and requirements.
