Care Coordination Professional

The Care Coordination Professional will play a vital role in ensuring the smooth delivery of home care services to PACE participants. This position requires a strong understanding of the referral process, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Duties and Responsibilities- Coordinate home care services, including scheduling appointments and communicating with vendors.
- Maintain accurate records of participant information, referrals, and service delivery.
- Collaborate with internal staff and external vendors to ensure seamless service delivery.
- Provide exceptional customer service to PACE participants, families, and caregivers.

Estimated Salary: $72,000 per annum, based on location and experience.
