We are seeking an experienced and compassionate Professional Ambulatory Nurse Coordinator to join our team in Boston, Massachusetts.

Job Summary

This is a 13-week contract position that requires strong ambulatory care skills, particularly in pulmonary, thoracic, and transplant settings. As a post-lung transplant nurse coordinator, you will be responsible for managing the ambulatory and clinic needs of post-lung transplant patients. You will work collaboratively with a multidisciplinary team to provide high-quality patient care.

Key Responsibilities:
  • Manage patient flow and coordinate care in the ambulatory setting
  • Conduct thorough assessments and develop personalized care plans
  • Collaborate with healthcare teams to ensure seamless transitions of care
  • Provide education and support to patients and their families
