RN Case Manager

The RN Case Manager will play a vital role in ensuring the delivery of high-quality patient care. This position will be responsible for assessing treatment needs, developing and monitoring treatment plans, and facilitating interdisciplinary approaches to care. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Key Responsibilities
* Assess patient needs and develop individualized treatment plans
* Coordinate and provide care that is safe, timely, effective, efficient, and patient-centered
* Facilitate interdisciplinary approaches to care, including collaboration with healthcare teams and families
* Monitor and evaluate treatment plans and progress, making adjustments as needed
Requirements
* Current California RN license
* Three to five years of experience in an acute care hospital setting (medical/surgical preferred)
* Previous one to two years of experience as a Case Manager
* Proficiency in typing and computer skills, including Microsoft Office
* Ability to write and communicate professionally
Preferred Qualifications
* Knowledge of University and medical center organizations, policies, procedures, and forms
* Broad knowledge of Medicare, Medi-Cal, and insurance guidelines
* Critical care experience (for Resource Case Manager)
