Job Title: Residential Medication Specialist

AZE Pharmacy seeks a licensed pharmacist to work as a Residential Medication Specialist in Brooklyn, NY.

Responsibilities:
  • Medication Therapy Management: Develop and implement individualized medication plans for patients.
  • Patient Education: Educate patients and their families about medication use, potential side effects, and interactions.
  • Collaboration: Collaborate with healthcare providers to ensure seamless communication and coordination of care.

Requirements:
  • Valid Pharmacist License: A valid license to practice pharmacy in the state of New York.
  • Doctor of Pharmacy Degree: A Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Experience: 3-5 years of experience in a pharmacy setting, preferably in a leadership role.

Salary:

$100,000 - $125,000 per year, depending on experience.
