Care Manager RN

About the Role
We are seeking a skilled RN Care Manager to join our team at Providence Newberg Medical Center. As a RN Care Manager, you will play a vital role in providing high-quality patient care and ensuring a smooth transition for patients from hospital to home.
Key Responsibilities
- Proactively develop and implement transition plans in coordination with the patient/family and interdisciplinary team.
- Support the interdisciplinary team as needed to develop transition plans for patients who do not require care in a non-home setting or in-home services post-hospitalization.
- Work with patients/families/caregivers and the interdisciplinary team to enable patients to progress towards care goals and be prepared for timely transition to the appropriate level of care in the community.
- Apply clinical expertise and knowledge of nursing, care management, community resources and utilization management to establish transition plans.
Requirements
- Nursing degree/diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- Oregon Registered Nurse License upon hire.
- 2 years Acute care hospital or community based experience.
- 1 year experience with electronic medical records or documentation systems.
- 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.
