Care Manager RN

We are seeking a skilled RN Care Manager to join our team at Providence Newberg Medical Center. As a Care Manager, you will play a vital role in providing high-quality patient care and ensuring a smooth transition from hospital to home.
Key Responsibilities- Develop and implement transition plans in coordination with patients, families, and interdisciplinary teams.
- Provide education and support to patients and families to enable them to progress towards care goals.
- Collaborate with the continuum care team to ensure effective care coordination and resource utilization.
- Apply clinical expertise and knowledge of nursing, care management, and community resources to establish transition plans.
- Nursing degree/diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- Oregon Registered Nurse License upon hire.
- 2 years Acute care hospital or community-based experience.
- 1 year experience with electronic medical records or documentation systems.
- 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience.
