Key Responsibilities:
- Develop and implement transition plans in coordination with patients, families, and interdisciplinary teams.
- Provide care coordination, education, and community care referrals to patients and families.
- Collaborate with the continuum care team to ensure effective care planning and monitoring.
- Apply clinical expertise and knowledge of nursing, care management, and community resources to establish transition plans.
Requirements:
- Nursing degree/diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- 2 years Acute care hospital or community-based experience.
- 1 year experience with electronic medical records or documentation systems.
- 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience.
