RN Care Manager

We are seeking a skilled RN Care Manager to join our team at Providence Medical Group. As a RN Care Manager, you will provide high-quality, patient-centered care to our most medically fragile patients and their families/caregivers.
Key Responsibilities- Provide across-the-continuum, customized care coordination services and management of transitions of care for patients with complex medical needs.
- Develop and implement patient-centered care plans that promote self-management and independence in the home/community setting.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care transitions and optimal patient outcomes.
- Conduct home visits to assess patient needs and provide education on disease management, medication adherence, and self-care.
- Work closely with our team of physicians to provide IV fluids, IV medications, EKGs, and assessments in the home setting.
- Oregon Registered Nurse License upon hire.
- 2 years Clinical medical-surgical, acute care or long-term care experience that also includes:
- 1 year in community-based/ambulatory care.
- 1 year in care of the frail elderly.
- Bachelor's Degree in Nursing.
- Home health, hospice, palliative care, managed care, or skilled nursing facility background.
- 1 year Care management of diverse patient populations.
