Role Summary

The Clinical Pharmacist is responsible for delivering specialized pharmaceutical services aimed at enhancing medication use and therapeutic outcomes for patients. This role involves active participation in multidisciplinary patient-care rounds, thorough evaluation and verification of medication orders, and overseeing drug distribution and technician activities. The Clinical Pharmacist also engages in clinical services such as medication histories, pharmacokinetic dosing, and patient counseling to ensure optimal medication therapy results.

Key Responsibilities

The primary responsibilities include:

  • Conducting medication order evaluations for appropriateness and effectiveness.
  • Providing support for drug distribution and supervising pharmacy technicians.
  • Engaging in clinical interventions and managing formulary processes.
  • Serving as a resource for nursing and physician inquiries related to medications.
  • Collaborating with pharmacy leadership to develop and implement clinical pharmacy services.

Comprehensive Pharmaceutical Care

Delivering thorough pharmaceutical care involves:

  • Collaborating with healthcare providers during patient care rounds.
  • Reviewing medication orders for accuracy and efficacy.
  • Documenting pharmacy interventions in electronic medical records.
  • Monitoring drug therapy for appropriateness and potential interactions.
  • Providing consultations on pharmacokinetics and dosing recommendations.

Cost-Effective Drug Therapy

Promoting rational drug therapy includes:

  • Participating in formulary management and therapeutic interchanges.
  • Assisting in medication procurement and verification processes.
  • Documenting clinical consultations and cost avoidance efforts.

Professional Development

Facilitating scholarly activities involves:

  • Providing guidance to pharmacy technicians and precepting pharmacy students.
  • Participating in departmental research and local, regional, or national organizations.

Policy Compliance

Executing hospital and pharmacy policies entails:

  • Collaborating with pharmacy leadership on policy development.
  • Functioning as a role model and educational resource for pharmacy staff.
  • Completing necessary training and demonstrating competency in sterile compounding.

Effective Communication

Utilizing advanced communication skills to:

  • Establish collaborative relationships with healthcare team members.
  • Articulate recommendations clearly to patients and staff.

Qualifications

Education and Licensure:

  • Graduated from an accredited College of Pharmacy.
  • Licensed or eligible for licensure as a pharmacist.
  • Current board certification required within two years of hire.
  • Basic Life Support and Advanced Cardiac Life Support certification required.

Experience:

  • Post-Graduate Year 1 Pharmacy Residency or three years of clinical experience required.
  • Post-Graduate Year 2 Pharmacy Residency preferred.
