Position Overview

Amaray Care is seeking a dedicated Registered Nurse to fulfill a vital role in patient care. This position entails overseeing the treatment of individuals with diverse medical needs. The nurse will be responsible for observing and documenting medication administration, monitoring patient progress, and providing education to both families and patients as necessary.

Currently, we are looking for nurses who are willing to travel to various locations within the PG County area for in-person evaluations. The ideal candidate will demonstrate proficient skills in assessing both potential and current clients' conditions. Responsibilities include supervising caregiver-administered client care in alignment with the client's care plan, ensuring compliance with agency policies, and adhering to Medicare and Medicaid regulations. This role reports directly to the Director of Nursing (DON) and the Operations Manager.

Key Qualifications

1. Must possess a current Registered Nurse (R.N.) license.

2. Ability to work positively and collaboratively with patients, families, and staff.

3. Reliable transportation is essential.

Core Responsibilities: The individual in this role must be capable of performing the following essential functions with or without reasonable accommodations:

1. Conduct initial home care patient visits and regularly re-evaluate patient needs and progress.

2. Initiate care plans as directed by physicians.

3. Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for home care patients.

4. Observe, assess, and document patient symptoms and reactions.

5. Monitor patient progress and reactions to treatments.

6. Educate patients and caregivers on disease processes, medications, and individualized treatment plans.

7. Provide guidance on techniques for effective in-home health care.

8. Coordinate patient services effectively.

9. Supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).

10. Notify physicians and relevant personnel of any changes in patient conditions.

11. Follow the agency's approved policy and procedure manual for skill outlines.

12. Discharge patients from skilled nursing services when criteria are met.

13. Collaborate with clinicians to ensure coordinated care.

14. Maintain clinical records in accordance with policy and procedures.

15. Enhance knowledge and skills through ongoing education and training.

16. Provide on-site supervision of LPNs and CNAs.

17. Comply with all Medicaid and Medicare regulations.
