Position Overview

Amaray Care is seeking a dedicated Registered Nurse to fulfill a vital role in our organization. This individual will be responsible for managing the care of patients with diverse health issues. Key responsibilities include monitoring medication administration, documenting patient progress, and providing education to both patients and their families.

Role Requirements

We are currently looking for nurses who are willing to travel within the PG County area for in-person evaluations. The ideal candidate will possess strong assessment skills to evaluate both potential and existing clients' health conditions. Responsibilities include supervising caregiver services in alignment with each client's care plan while ensuring adherence to agency policies and Medicare and Medicaid regulations. This role will report directly to the Director of Nursing and the Operations Manager.

Qualifications

Essential Qualifications:

  • Must be a licensed Registered Nurse (R.N.) with an active license.
  • Ability to work effectively with patients, families, and healthcare staff.
  • Access to reliable transportation.
Key Responsibilities

The individual in this role will be expected to perform the following essential functions:

  • Conduct initial home care visits and regularly reassess patient needs.
  • Initiate care plans as directed by physicians.
  • Complete necessary documentation for admissions, transfers, recertifications, and discharges.
  • Observe and document patient symptoms and reactions.
  • Monitor patient progress and responses to treatment.
  • Educate patients and caregivers on health conditions, medications, and care plans.
  • Coordinate services for patients effectively.
  • Supervise LPNs and CNAs in patient care.
  • Communicate changes in patient conditions to the appropriate personnel.
  • Ensure compliance with agency policies and procedures.
  • Participate in case conferences to enhance care coordination.
  • Maintain clinical records in accordance with established policies.
  • Engage in ongoing professional development through training and education.
