We are seeking a skilled Social Worker to join our Hospice and Palliative Care team at MJHS. As a Social Worker, you will provide psychosocial support and other Social Work services to Hospice patients and their families in various settings, including inpatient, home care, and nursing home settings.
Key Responsibilities- Develop and implement social work plans of care for assigned patients, including goals and recommendations based on patient/family needs.
- Provide psychosocial support and other Social Work services to Hospice patients and their families in various settings.
- Collaborate with interdisciplinary teams to ensure comprehensive care for patients and their families.
- Develop and maintain relationships with community resources and organizations to support patients and their families.
- Master's degree in Social Work from an accredited program.
- Minimum of one year experience in Social Work in a healthcare setting.
- New York State MSW licensure.
- Valid NYS Driver's license.
- Excellent organizational, communication, and listening skills.
- Ability to input clinical data proficiently.
- Bilingual: English and Mandarin or Cantonese.
