Key Responsibilities:
- Providing personalized care and services to residents in accordance with their service plans.
- Assisting residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating.
- Monitoring resident health and reporting unusual symptoms or concerns to supervisors.
- Documenting tasks and medication administration in the resident electronic medical record.
- Collaborating with the Clinical Care Team to ensure a person-centered care environment.
Requirements:
- High School Diploma or GED.
- Must be 18+ years old.
- Responsible, reliable, and professional.
- Certified Nursing Assistant (CNA) preferred but not required.
