Key Responsibilities:

  • Providing personalized care and services to residents in accordance with their service plans.
  • Assisting residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating.
  • Monitoring resident health and reporting unusual symptoms or concerns to supervisors.
  • Documenting tasks and medication administration in the resident electronic medical record.
  • Collaborating with the Clinical Care Team to ensure a person-centered care environment.

Requirements:

  • High School Diploma or GED.
  • Must be 18+ years old.
  • Responsible, reliable, and professional.
  • Certified Nursing Assistant (CNA) preferred but not required.
