Job Summary

Harbor, Inc. is seeking a highly skilled Intensive Case Manager to join our Benefit Bridge Program team. As a key member of our team, you will provide comprehensive support to individuals in developing personalized goals, connecting them with community resources, and facilitating participation in Benefit Bridge services that promote economic employment pathways to increased self-sufficiency.

Full-time position available.

Key Responsibilities
  • Develop and implement individualized service plans (ISPs) for participants, reflecting their unique needs and goals.
  • Provide vocational rehabilitation services, including job development, job coaching, and support in exploring employment opportunities.
  • Collaborate with participants, family members, and other stakeholders to identify and address bio-psychosocial needs.
  • Ensure accurate and timely documentation of services provided, including activity/progress notes, incident reports, and other relevant documentation.
  • Meet or exceed unit expectations for billable services.
