We are currently seeking a Nurse Case Management Specialist to become part of our dedicated team:
  • Schedule: Monday to Friday, 8AM-4:30PM

  • The Case Manager is essential in orchestrating discharge planning for hospitalized patients and executing various case management responsibilities as necessary.

Required Qualifications:
  • Current Registered Nurse license in Texas
  • CCM certification is preferred
Experience Requirements:
  • One to three years of experience in Case Management
  • Strong communication and leadership abilities
  • Familiarity with industry standards and basic computer proficiency
